Drake Medox is seeking a Registered Nurse with Aged Care experience to take up the role of Care Co-Ordinator in a state of the art facility which is one of several in a privately onwned group. The successful applicant will need to be a Registered Nurse with current AHPRA registration and post graduate experience in Aged Care. Managerial qualificaions and/or experience will be highly regarded.

The role will be responsible for:

• Ensuring compliance to the Commonwealth Goverment Standards in Aged Care and OH&S standards of the Facility
• Provide guidance on staffing levels, education, clinical direction and care planning for the Facility
• Ensureing the well being of residents by liaising between residents, relatives and the Facility
• Assist AINs to ensure residents needs are being met
• Co-ordinate any community care packages attached to the Facility.
• Demonstrate open and effective communication with the Facility
• Monitor ACFIs and ensure completion by the due date
• Participate and promote on-going staff development

This role will be a permanent role following successful probationary period of 3 months.
